ePlans to Promote Healthy Weight Use Cases

Supporting the creation of healthy weight plans through the electronic health record.

Childhood obesity is the most common chronic medical condition in children. Prevention and treatment of childhood obesity is based on promotion of healthy nutrition and physical activity behaviors and ongoing surveillance of weight status throughout childhood, particularly in primary care centers. The electronic health record (EHR) is widely recognized as a critical tool in this process. The ePlans to Promote Healthy Weight Use Cases serve to specify the essential information that must be captured and the key functions that must be performed by EHRs so that primary care teams and families can develop effective healthy weight plans together.

The use cases were developed as part of the e-Plans to Promote Healthy Weight project.
